Gather the product identifiers, current labels and instructions, evidence for your principal claims, and information about use, accessibility and support. The entry checklist brings the requirements together.

Create an account and verify your email. The application is divided into ten steps, from organisation and product details to evidence, files and declarations. Save each step and return whenever you need to.

Check the details and attachments, separate private evidence from the proposed public summary, and confirm the declarations. Submitting creates a receipt and fixes the application for review. You can follow its progress and answer questions in your account.

The product is considered against the Health Choice standard. You receive the outcome and reasons. If an award is made, you check the public product details before the record and free digital materials are released.
